ROLE OF NIGELLA SATIVA (Black Cumin Seed) IN OPIOID DEPENDENCE

Abstract
Opioid dependence is a chronic disorder that produces changes in brain pathways that remain long after the patient stops taking the drug. These protracted brain changes put the dependent person at greater risk of relapse. Detoxification can be successful in cleansing the person of drugs and withdrawal symptoms; it does not address the underlying disorder, and thus is not the adequate treatment. Maintenance with methadone or naltrexone is the usual practice in the long-term management of opioid dependence but both drugs have their own disadvantages because no single medication is appropriate for every individual for treating their opioid dependence, it is important that clinicians have a variety of the therapeutic agents available to them.
Calcium channel blockers, such as verapamil, diltiazem, nifedipine, nimodipine, and felodipine are useful drugs being used in cardiovascular disorders, such as hyper-tension, arrhythmias, and ischaemic heart disease. Research on calcium channel blockers has proved their therapeutic potential in a variety of disorders such as asthma, diarrhoea, premature labour, and diseases of central nervous system such as epilepsy, and opioid dependence. Modern drugs are not only expensive and beyond the reach of majority of the population of world but also have multiple side effects. Hence there is a need to explore such drugs from indigenous sources and to observe if combination of desired therapeutic efficacy exists in nature.
Nigella Sativa is in use for the treatment of variety of ailments since ancient times. Research has based its many effects on their efficacy of blocking calcium channels. As calcium channels have been tried for the treatment of opioid dependence, so Nigella Sativa was used in this study. This study was carried out on 50 patients who were divided into two groups. Patients were admitted for 12 days and then weekly followed up for 12 weeks.
Each patient received placebo orally during day-1 and day-2 of admission. Thereafter Nigella Sativa was given to the patients from day-3 of admission to eighth week. Then the dose of each drug was tapered off during 9th and 10th weeks and then no treatment was given during last two weeks.
It was observed that Nigella Sativa showed a rapid improvement in signs and symptoms of acute opioid abstinence. It was also observed that Nigella Sativa prevented the development of significant craving and relapse. It is concluded that Nigella Sativa is effective in long term management of opioid dependence and it is suggested that further long term follow up studies may be designed with greater number of patients.

Origian Black cumin (carum bulbocastanum) is rarely available so Nigella sativa is widely used instead, in India Carum carvi is the substitute. {cumins are from Apiaceae or Umbelliferae (both names are allowed by the ICBN) family but Nigella sativa is from Ranunculaceae family} Black cumin (not Nigella sativa)seeds come as paired or separate carpels, and are 3-4mm long. They have a striped pattern of nine ridges and oil canals, and are fragrant(Ayurveda says"Kaala jaaji sugandhaa cha"=Black cumin seed is fragrant itself), blackish in colour, boat-shaped, tapering at each extremity, with tiny stalks attached; has been used for medicinal purposes for centuries, both as a herb and pressed into oil, in Asia, Middle East, and Africa. It has been traditionally used for a variety of conditions and treatments related to respiratory health, stomach and intestinal health, kidney and liver function, circulatory and immune system support, as analgesic, antiinflammatory, antiallergic, antioxidants, anticancer, antiviral and for general well-being.
In Islam, it is regarded as one of the greatest forms of healing medicine available. In the Unani Tibb system of medicine, Black cumin is regarded as a valuable remedy for a number of diseases.
The seeds have been traditionally used in the Middle East and Southeast Asian countries to treat ailments including asthma, bronchitis, rheumatism and related inflammatory diseases, to increase milk production in nursing mothers, to promote digestion and to fight parasitic infections. Its oil has been used to treat skin conditions such as eczema and boils and to treat cold symptoms. Many researchers have recently also studied it's reaction towards cancer, and it's said to have many anti-cancer conditions.^{[citation needed]} Its many uses have earned Black cumin seed the Arabic approbation 'Habbatul barakah', meaning the seed of blessing.

Scientific Research

Nigella sativa oil(not Black cumin seed oil) contains nigellone, which protects guinea pigs from histamine-induced bronchial spasms^[6] (perhaps explaining its use to relieve the symptoms of asthma, bronchitis, and coughing).

The presence of an anti-tumor sterol, beta-sitosterol, lends credence to its traditional use to treat abscesses and tumors of the abdomen, eyes, and liver.^[7]

Nigella sativa oil has been reported to be effective in treating opioid dependence.^[8]

Nigella sativa also has been reported to reduce calculi formation in rats' kidneys.^[9]

Nigella sativa oil containing melanthin, nigilline, damascene and tannin. Melanthin is toxic in large dosages and Nigelline is paralytic, so this spice must be used in moderation.

Besides scientific research, there are sayings of Mohammed that clearly underlines the significance of Nigella sativa. According to a hadith narrated by Abu Hurerah, he says that "I have heard Hazrat Muhammad saying that the black granules (Kalonji) is the remedy for all diseases except death." ^[10]

Thymoquinone and pancreatic cancer treatment

Researchers at the Kimmel Cancer Center at Thomas Jefferson University in Philadelphia have found that thymoquinone, an extract of Nigella sativa seed oil, blocked pancreatic cancer cell growth and killed the cells by enhancing the process of programmed cell death, (apoptosis). While the studies are in the early stages, the findings suggest that thymoquinone could eventually have some use as a preventative strategy in patients who have gone through surgery and chemotherapy or in individuals who are at a high risk of developing cancer.^[11]

Nigella sativa is one the most revered medicinal seeds in history. The best seeds come from Egypt where they grow under almost perfect conditions in oases where they are watered until the seed pods form. Black cumin seeds were found in the tomb of Tutankhamun. Though black cumin seeds are mentioned in the Bible as well as in the words of the Prophet Mohammed, they were not carefully researched until about forty years ago. Since this time, more than 200 studies have been conducted in universities.

The famous Greek physician Dioscorides used black cumin seeds to treat headaches and toothaches. Mohammed said that black cumin cures every disease but death itself. The reason might be found in the complex chemical structure of the seeds. These little seeds have over one hundred different chemical constituents, including abundant sources of all the essential fatty acids. Though it is the oil that is most often used medicinally, the seeds are a bit spicy and are often used whole in cooking—curries, pastries, and Mediterranean cheeses.

Nigella sativa seeds have very little aroma but are carminative, meaning they tend to aid digestion and relieve gases in the stomach and intestines. They aid peristalsis and elimination. The essential oil of black cumin is antimicrobial and helps to rid the intestines of worms.

Black cumin is regarded by many as a panacea and may therefore not be taken seriously by some, but for those inclined to dismiss folklore, it should be noted that these humble seeds have been found superior to almost every other natural remedy when used for autoimmune disorders, conditions in which patients suffer greatly because their own systems attack their bodies. Black cumin, especially when combined with garlic, is regarded as a harmonizer of the imbalance which allows immune cells to destroy healthy cells. The technical language to describe this property is "immunomodulatory action." The difference between black cumin and interferon is that there are no known side effects with black cumin when administered in normal dosages. The saying goes that the beauty of black cumin is their capacity to restore harmony.

The most dramatic results are achieved with asthma and allergies. These respond relatively quickly unless there is infection, in which case, the infection needs to be eliminated before the symptoms of immune weakness subside. Continued use for six months or longer tends to give outstanding results. For extreme fatigue, consider mixing some crushed seeds with some royal jelly.
With a seed containing so many constituents and having such a long ethnobotanical history, it is not surprising that many throughout the Mediterranean and Asia believe that black cumin is basically good for all that ails us. However, the claims are not outrageously far-fetched if one considers how complete the seeds are in terms of their many chemical constituents. Still, it is understandable that anyone who claims that something can do anything from increasing one's sperm count or increasing milk production in a nursing mother to relieving bronchial conditions such as asthma and bronchitis, is not taken seriously. One then wonders if the imagination of the poets has triumphed over the logic of scientists? Just remember: those paying homage to the black seeds of the Egyptian oases were praising the capacity of the seeds to restore normalcy, not cure. This is not unimaginable if the nutrients are sufficient to correct deficiency conditions.

Protects Healthy Cells and Stimulates Production of Natural Interferon

The first major study of Nigella sativa in cancer prevention and treatment was performed by scientists at Cancer Immuno-Biology Laboratory of Hilton Head Island, South Carolina. They concluded that a healthy immune system will detect and destroy cancer cells before the cancer endangers the patient. The immune system does this by supporting increased production of immune cells, bone marrow cells, and B-cells that produce antibodies. Black cumin stimulates neutrophil activity. These are the short-lived immune cells that are normally found in bone marrow but mobilized into action when there is a bacterial infection. Extracts of black cumin have also been shown to modulate production of interleukins, a quality it shares with some other highly revered herbs: ginseng, astragalus, mistletoe, garlic, and cat's claw.

In animal studies, while none of the subjects in the control group survived, two-thirds of the mice that had been given black cumin seed oil were still alive 30 days after deliberate efforts to cause cancer in the subject groups. Black cumin is particularly useful in aggressive cancers whose growth depends on angiogenesis.

In vitro studies performed in Jordan and the United States have determined that the volatile oil is anti-leukemic. Studies performed in Spain as well as England found that the fixed oil is useful in the treatment of rheumatism and other inflammatory diseases. This property is attributed to thymoquinone which is as high as 25% in the Egyptian seed and missing entirely is some seeds.

Since D-ribose is ubiquitous in living matter, it is ingested in our diets. Such nutritional substances as brewers yeast are rich in RNA and are thus rich sources of D-ribose. Some recent research suggests that supraphysiological amounts of this sugar may have cardioprotective effects, particularly for the ischemic heart.
D-ribose is a sweet, solid, water-soluble substance that is also known as alpha-D-ribofuranoside. L-ribose does not have biological activity. D-ribose is sometimes referred to as just ribose. Supplemental D-ribose is produced from the fermentation of corn syrup.
Ribose is used to generate ATP (adenosine triphosphate). In theory, supplementing your diet with additional ribose should increase the rate at which ATP is generated, leading to an improvement in exercise performance and faster muscle growth. ATP is constantly being broken down and "recreated". In the process, it provides energy for every move you make and every chemical reaction that occurs in human body. The adenine portion of adenine triphosphate consists of one molecule of adenine and the five-carbon sugar, ribose. The triphosphate portion of ATP consists of three phosphate molecules.
Energy is released when one of those phosphates is broken off from ATP. The compound then becomes adenosine diphosphate (ADP), which consists of adenosine and two phosphate molecules. ADP becomes adenosine monophosphate (AMP) when another phosphate molecule is broken off. The ratio between ATP, ADP and AMP is critical in regulating the energy content of a cell. However, during high-intensity exercise, the cell is unable to "recreate" ATP fast enough. Under these conditions, the concentrations of ADP and AMP rise, leading to a reduction in the "energy charge" of the cell. If ATP use continues to exceed the rate at which it can be generated, nucleotides are lost from the cell in an attempt to restore the ratio between ATP, ADP and AMP. During anaerobic metabolism, in an attempt to keep up with cellular energy demands, 2 ADP's combine to form 1 ATP and 1 AMP, by way of a metabolic process called the myokinase reaction. As a result, AMP builds up in the cell, and disturbs the ratio of ATP to ADP and AMP, which the cell works to keep in check. To do this, AMP concentrations must be lowered by degrading AMP to simpler end products, which are washed out of the cell and lost forever. The final result, is a dramatic decrease in the adenine nucleotide pool. In fact, the adenine nucleotide pool can decrease by as much as 50% in heart muscle following ischemia (diminished blood flow to tissues), and around 32% in skeletal muscle following high - intensity exercise. When the adenine nucleotide pool is reduced, performance and recovery are compromised very noticeably.
Ribose, when taken as a nutritional supplement, bypasses the slow conversion steps needed to recreate the adenosine nucleotide, and is readily available for the creation of more ATP. Replacing these lost adenine nucleotides can take several days, reducing the rate at which muscle fibers are repaired. Supplemental ribose can increase the speed at which these nucleotides are replaced, both at rest and during exercise. As such, there has been a great deal of interest in the potential of ribose supplements to boost muscular performance in sports.
